Half hour telephone consultation on any nutritional subject you may wish to discuss. Please note that diet formulations cannot be done by phone.
Information I need
Prior to the consultation I will ask you to complete a diet and health history form. This will provide me with the background information needed and help me understand your nutritional concerns before we schedule an appointment. Please complete and return this form to me within one week.
